Illuminate Colorado is seeking a Contract Medical Director for Family Connects Colorado to develop clinical guidelines and build relationships with healthcare providers. This role emphasizes collaboration and support for newborns and their families through an evidence-based home visitation model.
Key Responsibilities
- • Support development, annual review, and approval of Family Connects Colorado Clinical Guidelines
- • Participate in local site case conferences as needed, and twice a month Medical Director Office Hours
- • Support clinical implementation fidelity as identified through Family Connects Quality Assurance process and annual site performance reviews
- • Build relationships with clinical stakeholders in coordination with members of the Family Connects implementation sites and state leadership team
- • Enhance collaboration with pediatric and obstetric care practices to identify champions at area practices at state and local level
- • Educate and build partnerships with healthcare partners to ensure a comprehensive perinatal support system
- • Support hospital and health systems engagement efforts to optimize newborn enrollment through referring hospitals and medical providers, including developing workflows and streamline referral processes
- • Provide input and guidance on professional development for Family Connects Colorado nurse supervisors and nurses
- • Identify opportunities to align Family Connects within the existing public health and healthcare systems at a state and local level
- • Optimize newborn enrollment through referring hospitals, as needed
- • Be available to respond by phone/text to Director of Clinical and Community Engagement or Nursing Supervisor when urgent matters arise
- • Ensure referral back to the client’s primary provider, or other provider, as needed for additional intervention
- • Represent and advocate for the program model, goals, and fidelity across all sites in Colorado to help ensure goals are achieved
- • Must be able to fulfill these duties without conflict of interest due to other employment or professional/personal obligations
- • Other duties as assigned
Required
- • The Contract Medical Director must be an MD, DO, CNM (certified nurse midwife), PA (Physician Assistant), or NP (Nurse Practitioner)
- • At least five years of related experience in one of these three areas: OBGYN/midwifery or pediatrics
- • At least five years of related experience which may include implementing or supporting an evidence-based program or model, local public health leadership, and/or hospital systems engagement
- • Understanding and ability to navigate community health and hospital systems, including commonly used Electronic Health Records (EHR) systems
- • Passion for supporting collaborative processes and maternal child health initiatives through inclusive and culturally responsive practices
- • Commitment to advancing social and racial equity through collective action
- • Excellent oral and written communication skills
- • Demonstrated ability to manage collaborative projects, including balancing deadlines with community processes
- • Demonstrated ability to establish and maintain highly collaborative working relationships with individuals of diverse professional backgrounds, technical expertise, and lived experience
- • Comfort with ambiguity, shifting environments, and shared decision making
Preferred
- • Behavioral health experience in addition to this would be preferred but not required
- • The Contract Medical Director should collaborate with other specialists in areas that are not their specialty to cover all of the above

Role: Under the guidance of the Director of Clinical and Community Engagement, the Contract Medical Director for Family Connects Colorado will be responsible for development and endorsement of Family Connects Colorado clinical guidelines, building relationships with hospitals, and pediatric/OBGYN providers, as well as provide clinical guidance for Family Connects Colorado sites.
Family Connects is an evidence-based approach to supporting all newborns and their families. As a universal light touch home visitation model, Family Connects implementation is designed to create mechanisms, building on local community capacity and interests, that will provide all new parents, prenatally or at birth, systematic opportunities to discuss their concerns and be connected to the parent support resources available in their communities. Family Connects works in partnership with four current demonstration communities and is poised to scale.
Specific Duties Include:
Support the Director of Clinical and Community Engagement in providing Clinical Leadership for Family Connects Colorado, including:

Provide clinical support and consultation regarding patient care, including:
- Be available to respond by phone/text to Director of Clinical and Community Engagement or Nursing Supervisor when urgent matters arise
- Ensure referral back to the client’s primary provider, or other provider, as needed for additional intervention
Advocate for, and align with, Family Connects Colorado to support implementation of model fidelity according to Family Connects International’s guidelines and policies:
- Regardless of primary employer/hospital affiliation (if applicable), the Contracted Family Connects Colorado Medical Director, will represent and advocate for the program model, goals, and fidelity across all sites in Colorado to help ensure goals are achieved
- Must be able to fulfill these duties without conflict of interest due to other employment or professional/personal obligations

Compensation & Schedule:
Contract role (approximately 8 hours per month). Not eligible for benefits.
Compensation: $120-150/hour based on experience and credentials
