Role Overview: Post-Acute Care SNFist Nursing Home Physician

This role will be responsible for managing patient care at post-acute skilled nursing home facilities. It involves working with and supervising a team of nurse practitioners over multiple facilities in Central Florida and maintaining the highest standards of patient care and operational efficiency.

Role Description

This is a full-time remote role for a Post-Acute SNFist Nursing Home Physician. The physician will be responsible for providing medical care to nursing home residents, managing chronic conditions, conducting assessments, developing treatment plans, and coordinating with the nursing staff. The role also includes monitoring patient progress, adjusting treatments as necessary, and ensuring comprehensive care documentation.

Qualifications

Board certification in Family Medicine, Internal Medicine, or Geriatrics Experience in managing chronic conditions in a nursing home or post-acute care setting preferred Skilled in conducting assessments, developing treatment plans, and coordinating care Familiarity with electronic health records (EHR) and telehealth platforms Excellent communication and interpersonal skills Ability to work independently and remotely Dedication to patient-centered care and a holistic approach to health
