About The Role

We are seeking warm-hearted, board-certified physicians (MD/DOs) to partner with IMPaCT Care as Physician Partners to play a short-term, high-impact role, helping identify health conditions and care gaps, stabilizing medical concerns, and connecting patients to longer-term primary care solutions.

As a Physician Partner, You Will

Dedicate 5–10 hours per week to real-time telehealth visits with patients referred by CHWs including consults, prescribing medications when appropriate, and providing clinical guidance. Collaborate with CHWs to evaluate patient needs and determine eligibility for Community Health Integration services. Provide clinical consultation and oversight to CHWs as they support patients with social needs and healthcare navigation. Promote preventive care and overall well-being through education and evidence-based strategies. Maintain accurate and up-to-date patient records, including reviewing and affirming the documentation created by the patient’s CHW. Respond to patient questions, chart notes, and administrative messages as they transition to a primary care home. Provide occasional follow-up visits to address urgent or time-sensitive medical needs. Complete required training and adhere to company policies and best practices.

Requirements And Experience

Required Licensure: Active, unrestricted MD or DO license (NPs licenses also welcome) and current residence in the United States Required Experience: Minimum of 2 years of consecutive experience in family medicine, internal medicine, geriatrics, preventative medicine, or other primary care settings Commitment to Health Equity: Passionate about social medicine and addressing the social needs that impact health. Independence: Ability to operate effectively as an independent provider with strong diagnostic and problem-solving abilities. Clinical Collaboration: Experienced in providing clinical support and oversight to CHWs or other allied health professionals Telemedicine: Strong ability to connect with patients virtually and provide empathetic care; prior telehealth experience preferred Technical Proficiency: Skilled in navigating multiple computer screens and proficient in tools such as EMR, text expanders, Gmail, Google Calendar, Zoom, and scheduling platforms.
