Guthrie
The Assistant Chief Population Health Officer is a system-level executive leader responsible for centralizing, coordinating, and advancing value-based care and population health initiatives. This role provides strategic direction for infrastructure investments, IT, and integration of clinical and operational initiatives to improve health outcomes, patient and provider experiences, and reduce healthcare costs.
Key Responsibilities
- • Develop and lead the organization’s population health strategy, aligned with value-based care models and future population health needs.
- • Collaborate with executive leadership to set goals for improving health outcomes, reducing costs, and addressing social drivers of health (SDOH) and health disparities.
- • Champion initiatives that foster interdisciplinary collaboration, team-based workflows, and patient engagement.
- • Represent Guthrie Clinic in external partnerships, including Medicare/Medicaid accountable care programs, Medicare Advantage, commercial payors, and community-based organizations.
- • Provide clinical guidance for outreach coordinators, care management, chronic disease programs, preventive care initiatives, and remote patient monitoring.
- • Oversee PCMH accreditation, ambulatory CDI specialists, and related projects.
- • Lead quality improvement efforts, utilization management strategies, and ensure evidence-based, culturally competent clinical protocols.
- • Collaborate with analytics teams to create and monitor quality, utilization, and risk adjustment performance metrics.
- • Use data-driven approaches to evaluate and target opportunities for improvement, embedding new workflows and solutions into reliable processes of care (e.g., Epic EHR optimization).
- • Align ambulatory and inpatient quality programs, working closely with the EVP Chief Quality Officer.
- • Ensure compliance with federal, state, and payer requirements related to population health and value-based care.
- • Oversee reporting for quality programs such as HEDIS and ACO metrics.
- • Develop and maintain financial tracking of incentive plans; collaborate with finance for appropriate allocation of funds.
- • Engage physicians and care teams in population health initiatives through education, performance feedback, and ongoing communication.
- • Partner with leaders in the Guthrie Pulse Center, home health, and national organizations to integrate best practices and remote monitoring into clinical care.
- • Motivate and influence others to raise system performance on internal and externally reported measures.
- • Oversee employment actions including hiring, performance reviews, disciplinary actions, and terminations.
- • Plan, schedule, and organize work for teams, ensuring efficient utilization of personnel and resources.
- • Prepare reports summarizing activities and performance metrics related to process improvement initiatives.
- • Identify organizational risks and provide solutions related to quality incentive programs.
- • Other duties as assigned.
Required
- • Physician License in Pennsylvania and New York required; Board Certification and current competence in relevant fields.
- • Alternatively, Nursing license with bachelor’s in nursing and master’s degree in population health or related field.
- • Minimum of 5 years of leadership experience in an integrated health system.
- • At least 3 years of clinical experience required.
- • Experience in quality improvement, clinical data reporting, operational redesign, and value-based care programs.
- • Experience with quality chart reviews, abstracting, coding, electronic medical records, and team management.
- • Strong leadership, communication, and interpersonal skills.
- • Deep understanding of healthcare delivery systems, value-based payment models, and SDOH.
- • Proficiency in EHRs and population health management tools.
- • Ability to lead multidisciplinary teams and drive change in complex environments.
- • Organizational, analytical, and strategic planning skills.

- • Additional coding knowledge or certification desirable.
