Medical Director & Skilled Nursing Facility Primary Care Physician (MD/DO)
This role combines providing on-site primary care services to skilled nursing facility residents with functioning as Medical Director. The position includes leadership development, participation in Certified Medical Director (CMD) training, and managing a predictable clinical volume supported by NP/PA coverage and structured after-hours telehealth.
Key Responsibilities
- • Provide on-site primary care services to skilled nursing facility residents
- • Evaluate residents for significant changes in condition
- • Collaborate with interdisciplinary care teams and facility leadership
- • Supervise and support NP/PA clinicians providing routine and regulatory care
- • Participate in a defined, structured after-hours call schedule
- • After-hours clinical issues managed via telehealth by the physician or NP/PA on call
- • Function as Medical Director for assigned skilled nursing facilities
- • Participate in Certified Medical Director (CMD) training
- • Gain experience in Quality Assurance & Performance Improvement (QAPI)
- • Gain experience in Regulatory compliance and survey readiness
- • Gain experience in Interdisciplinary leadership
Required
- • MD or DO
- • Board certified or board eligible in Internal Medicine, Family Medicine, Geriatrics, or Emergency Medicine
- • Active Tennessee medical license (or ability to obtain)
- • Interest in post-acute care, leadership, and community-based medicine
Preferred
- • Certified Medical Director (CMD) training fully funded
- • CMD certification is not required to start
