Conduct in-person, mobile, and virtual visits, manage conditions with a longitudinal, whole-person approach, and coordinate care as the clinical anchor for a dedicated care pod.
Key Responsibilities
- • Provide high-quality, relationship-based primary care to a small, intentional panel of members across multiple life stages
- • Serve as a private health steward guiding members through preventive care, acute needs, chronic conditions, and long-term health planning
- • Conduct in-person, mobile, and virtual visits, meeting members where they are at home, work, or in the community when appropriate
- • Manage acute and chronic conditions with a longitudinal, whole-person approach
- • Order, interpret, and contextualize laboratory testing, imaging, and diagnostics to inform thoughtful, personalized care plans
- • Serve as the clinical anchor for the care pod, coordinating with specialists and ensuring continuity across the healthcare ecosystem
- • Lead and collaborate with a dedicated care pod, including a registered nurse and member care coordinator, fostering a culture of excellence and accountability
- • Educate and empower members to understand their health, make informed decisions, and build sustainable habits
- • Monitor outcomes over time, adjusting care plans based on data, experience, and evolving member goals
Required
- • MD or DO; board-certified in Family Medicine
- • Minimum of 5 years of experience practicing as a licensed physician
- • Texas medical license (or ability to obtain)
- • Comfort with mobile and non-traditional care settings
- • Has a proven track record of delivering extraordinary patient care and service
- • Excellent ability to multitask and work effectively under pressure
- • Intellectual humility and the ability to communicate compassionately

What we are looking for:
