We care for a small, intentional panel so we can know our members well, show up when it matters, and practice medicine the way it was meant to be practiced. We are a family medicine led concierge practice built on private health stewardship- partnering with individuals and multi-generational families to guide health over time. We provide comprehensive primary care for adults and children, emphasizing continuity, proactive planning, and thoughtful coordination across every stage of life. Our model prioritizes depth, trust, and long-term outcomes, supported by a dedicated care pod designed to protect both the patient experience and physician well-being.

What will you do

• Provide high-quality, relationship-based primary care to a small, intentional panel of members across multiple life stages
• Serve as a private health steward guiding members through preventive care, acute needs, chronic conditions, and long-term health planning
• Conduct in-person, mobile, and virtual visits, meeting members where they are at home, work, or in the community when appropriate
• Manage acute and chronic conditions with a longitudinal, whole-person approach
• Order, interpret, and contextualize laboratory testing, imaging, and diagnostics to inform thoughtful, personalized care plans
• Serve as the clinical anchor for the care pod, coordinating with specialists and ensuring continuity across the healthcare ecosystem
• Lead and collaborate with a dedicated care pod, including a registered nurse and member care coordinator, fostering a culture of excellence and accountability
• Educate and empower members to understand their health, make informed decisions, and build sustainable habits
• Monitor outcomes over time, adjusting care plans based on data, experience, and evolving member goals

What we are looking for:

• MD or DO; board-certified in Family Medicine
• Minimum of 5 years of experience practicing as a licensed physician
• Texas medical license (or ability to obtain)
• Comfort with mobile and non-traditional care settings
• Has a proven track record of delivering extraordinary patient care and service
• Excellent ability to multitask and work effectively under pressure 
• Intellectual humility and the ability to communicate compassionately
