Key Responsibilities
- • Provide on-site primary care services to skilled nursing facility residents
- • Evaluate residents for significant changes in condition
- • Collaborate with interdisciplinary care teams and facility leadership
- • Supervise and support NP/PA clinicians providing routine and regulatory care
- • Participate in a defined, structured after-hours call schedule
- • Manage after-hours clinical issues via telehealth by the physician or NP/PA on call
- • Function as Medical Director for assigned skilled nursing facilities
- • Participate in Certified Medical Director (CMD) training
- • Gain experience in Quality Assurance & Performance Improvement (QAPI), regulatory compliance and survey readiness, and interdisciplinary leadership
Required
- • MD
- • Board certified or board eligible in Internal Medicine, Family Medicine, Geriatrics, or Emergency Medicine
- • Active Pennsylvania medical license (or ability to obtain)
- • Interest in post-acute care, leadership, and community-based medicine

Position Overview
We are seeking a Physician (MD) to serve in a combined role providing on-site skilled nursing facility primary care while functioning as Medical Director, supported through participation in Telos' Certified Medical Director (CMD) training.
