Physician Home Care

Provide hospice care to patients and families in home settings using an interdisciplinary team approach. Build meaningful relationships with patients and families, create patient-centered care plans, advocate for patients, and educate patients, caregivers, and communities on compassionate, dignified end-of-life care.

Required
  • licensed to practice medicine or osteopathy in the state in which their program operates
  • board-certified or board-eligible in hospice and palliative medicine, internal medicine, family practice, or a related specialty
  • familiar with the principles of hospice
  • registered with the DEA

What You’ll Do

As a home care hospice p hysician, you will use your compassion, communication skills and deep expertise to build meaningful relationships with patients and families, providing the highest- quality care at the most challenging point in life. You will see the person behind each prognosis and create care plans centered around the patient’s w ishe s, needs, and beliefs, often working in concert with the patient’s primary care physician.

Manage approximately 45-60 patients in their home settings via an interdisciplinary team approach. Make visits in-person and via telehealth. Participate in Weekly team IDG meetings.

With your strong sense of commitment, y ou will advocate for your patients, discuss ing goals for their care while address ing their hopes and fears. Yo u will live the hospice mission, s eeking teachable moments everywhere, and educat e patients, caregivers, and the communi ties you serve on the benefits of compassionate, dignified end- of- life care.

WHERE YOU’LL WORK

Each day is different. You will spend most of your time caring for hospice patients and families wherever they call home, whether that means a private residence or an assisted living/skilled nursing facility.

Once a week, you’ll visit your local program office to discuss cases with members of your interdisciplinary team, including social workers, chaplains, and nurses. All of these roles collaborate to provide holistic support for the patients and families we serve.

What’s Expected From You

VITAS physicians must be licensed to practice medicine or osteopathy in the state in which their program operates. In addition to being board-certified or board-eligible in hospice and palliative medicine, internal medicine, family practice, or a related specialty, you should be familiar with the principles of hospice and registered with the DEA.
