Provide high-quality, preventive-focused primary care through telehealth for Medicare patients in a low-acuity, standardized environment. This role focuses on routine telehealth visits, chronic care management, care planning, documentation, and collaboration with an integrated care team to help seniors live healthier lives.
Key Responsibilities
- • Provide routine telehealth visits with Medicare patients (E/M and Chronic Care Management)
- • Create and manage personalized care plans for ongoing conditions
- • Collaborate with our integrated care team (nurse case managers, NPs, CHWs)
- • Ensure thorough documentation in our EHR and compliance with quality measures
- • Engage patients in preventive care to reduce avoidable complications
Required
- • MD or DO
- • Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
- • Active medical license
- • Credentialed with major plans: UHC, Aetna, Humana
- • Strong interest in preventive care, senior health, or chronic condition management
- • Tech-savvy and comfortable practicing via telehealth
- • Collaborative, mission-driven mindset with excellent communication skills
Preferred
- • Multi-state licensure a plus
