Provides care for patients with acute and chronic illnesses in older adults, with full accountability for patient care and outcomes. The role includes assessment, diagnosis, treatment, management, education, health promotion, care coordination, documentation, and collaboration with specialists, hospitalists, and transitional care resources as needed.
Key Responsibilities
- • Independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.
- • Takes full accountability for patient care and outcomes and appropriately seek consultation from specialists when needed, though will still stay involved in, and be responsible for, the detailed care of the patient.
- • Engages with the hospitalist whenever one of their patients is in the hospital (regardless of whether the hospitalist works for ChenMed or not).
- • Responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs.
- • For patients that are unable to come to the office—in hospital, SNF, LTC or homebound, engages with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources that may be available depending on the market.
- • Leads Super Huddle (SH) and Transforming Care Meeting (TCM) weekly.
- • Supports Center Manager/Center General Manager if the center clinical leader and/or market clinical leader is not available, based on guidance from Market Chief Medical Officer.
- • May work at other Dedicated Senior Medical Center sites in Orange County.
Required
- • Medical Degree or foreign equivalent
- • Board Certified/Board Eligible (BC/BE) in Family Medicine or Internal Medicine
- • Must possess a Florida State Medical license
- • Must possess a Drug Enforcement Administration (DEA) license

Offer Salary: $214,700/year - $248,000/year

Pay Range
$221,141 - $315,915 Salary
The posted pay range represents the base hourly rate or base annual full-time salary for this position. Final compensation will depend on a variety of factors including but not limited to experience, education, geographic location, and other relevant factors. This position may also be eligible for a bonuses or commissions.
