This role provides high-quality, personalized medical care to senior and medically complex patients through in-home visits, skilled nursing facility visits, and telemedicine. Key Responsibilities
- • Conduct thorough in-home medical evaluations of senior and medically complex patients, including review of medical history, physical examination, and diagnostic tests as needed.
- • Conduct medical assessments of patients at skilled nursing facilities, including review of medical history, physical examination, and management of acute and chronic conditions.
- • Conduct virtual patient assessments, diagnose medical conditions, and provide treatment plans and follow-up care via telemedicine platforms.
- • Counsel and support senior patients on preventive care measures, vaccinations, and health-promoting lifestyle choices across in-home and telemedicine encounters.
- • Manage and monitor chronic conditions in senior patients to optimize health outcomes, oversee medication management, provide compassionate palliative care, and deliver patient and family education during in-home visits.
- • Collaborate effectively with interdisciplinary ‘UCSD at Home’–Population Health Services healthcare teams, including physicians, Advanced Practice Providers (APPs), nurses, social workers, and specialists, to ensure holistic and timely care.
- • Maintain accurate, up-to-date electronic health records (EHR) of patient encounters, treatment plans, and progress notes across all care settings.
Required
- • Medical degree (MD or DO) from an accredited institution.
- • Active and unrestricted medical license in California.
- • A current valid Driver's License in good standing and the ability to travel to patient's homes and Skilled Nursing Facilities throughout San Diego County.
- • Proven experience in primary care, home visits, and skilled nursing home care, particularly in geriatric medicine.
- • Strong interpersonal and communication skills, with the ability to build rapport with senior patients and their families.
- • Compassion, empathy, and cultural sensitivity when working with diverse senior populations.
- • Strong organizational and time management abilities.
- • Familiarity with telemedicine platforms and technology.
- • A commitment to providing patient-centered, in-home and telemedicine-based medical care.
Preferred
- • Board certification in Family Medicine, Internal Medicine, or Geriatrics is preferred.

As a Primary Care/Geriatrics Physician with ‘UCSD at Home’–Population Health Services, you will provide high-quality, personalized medical care to senior patients through in-home visits, skilled nursing facility visits, and telemedicine.
Key Responsibilities
- In-Home Assessments: Conduct thorough in-home medical evaluations of senior and medically complex patients, including review of medical history, physical examination, and diagnostic tests as needed.
- Skilled Nursing Facility Visits: Conduct medical assessments of patients at skilled nursing facilities, including review of medical history, physical examination, and management of acute and chronic conditions.
