This role is part of an expanding primary care program in Western North Carolina with a physician-centered model designed for long-term success. The work emphasizes team-based, whole-person primary care with RN support, embedded behavioral health, and coordinated care in an outpatient setting.

- • Team-based care with RN support for Transition of Care and Chronic Care Management
- • Support for addressing identified Social Determinants of Health
- • Behavioral health embedded within AdventHealth Primary Care
- • Whole-person focus with standardized dementia screenings and coordinated PT
- • Full-time physician for Weaverville, NC
- • Outpatient only
- • Very light clinic call

- • Board certified or within 5 years of board eligibility
