The Hospice Physician provides expert end-of-life care through physician oversight by telephone and telemedicine, symptom management, and hospice eligibility certification. The role partners closely with an interdisciplinary hospice team to support patients and families during serious illness and after-hours coverage.
Key Responsibilities
- • Provide physician oversight for hospice patients via telephone and telemedicine
- • Manage complex symptoms and adjust medications to maximize comfort and quality of life
- • Respond to admission and clinical consultation calls from nurses and admission staff
- • Certify hospice eligibility in accordance with Medicare guidelines
- • Collaborate closely with hospice nurses, nurse practitioners, social workers, chaplains, and other interdisciplinary team members
- • Provide after-hours physician coverage during assigned call rotations
Required
- • M.D. or D.O.
- • Board Certified or Board Eligible in Internal Medicine, Family Medicine, Geriatrics, or a related specialty
- • Active California medical license
- • DEA registration
- • Excellent communication and clinical decision-making skills
- • 1 year Hospice or Palliative experience
Preferred
- • Hospice and Palliative Medicine board certification or fellowship training
- • Previous hospice, palliative care, geriatrics, hospital medicine, or complex chronic disease management experience

We are seeking a part-time, remote, Hospice Physician to join our physician team.
Location: Fully Remote with no routine in-person visits required (California Licensed Physician Required)
Schedule: 2.5 weekdays per week (Tuesdays and Thursdays, plus Wednesday mornings), plus on-call 1 weekend (day and night) approximately every 5–6 weeks
Position Type: Part-Time (more hours available if interested), contracted
The Hospice Physician is compassionate and collaborative in providing exceptional end-of-life care throughout SF, Sonoma and Marin Counties. This is a fully remote position with a flexible weekday schedule, making it an excellent opportunity for physicians seeking meaningful clinical work with work-life balance. The Physician is responsible for overall medical activities of patient care services and related medical administrative components. As a Hospice Physician, you will partner closely with the team to provide expert symptom management, determine hospice eligibility, and support patients and families during serious illness.
Hourly rate: $145 - $150 per hour (plus up to $400 per night during on-call rotation)
Supervisory Responsibilities: No direct reports
