Provide comprehensive primary care to patients across the full lifespan in a fully outpatient setting. Work with physicians, APPs, and care teams in a collaborative, technology-enabled environment to deliver value-based care, manage patient panels, and support care coordination and quality improvement.
Key Responsibilities
- • Provide comprehensive primary care to patients across the full lifespan
- • See 16–20 patients per day in a 100% outpatient setting
- • Deliver high-quality, patient-centered care aligned with value-based care principles
- • Collaborate with physicians, APPs, and care teams to coordinate care and improve outcomes
- • Manage an assigned patient panel, including preventive care and chronic disease management
- • Participate in care coordination and quality improvement initiatives
- • Utilize EMR to document care and manage patient records
- • Incorporate in-office procedures based on experience and interest (optional)
- • Provide after-hours call coverage for your patient panel (shared model may be available)
- • Engage in team-based care and contribute to a supportive, collaborative practice environment
Required
- • MD or DO degree from an accredited medical school (required)
- • Board Certified or Board Eligible in Family Medicine or Internal Medicine (required)
- • Hold an active, unrestricted Rhode Island medical license or be eligible to obtain one (required)
- • Commitment to patient-centered, value-based care (required)
- • Strong clinical judgment and ability to manage a broad range of primary care conditions
- • Excellent communication and collaboration skills
- • Comfort working in a team-based, technology-enabled care environment
Preferred
- • Interest in quality improvement and population health initiatives (preferred)
