Key Responsibilities

Required
- • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school
- • Completion of an accredited Family Medicine or Internal Medicine residency program
- • Board Certified or Board Eligible (FM or IM)
- • Active, unrestricted Florida medical license (or ability to obtain licensure prior to start)
- • Commitment to practicing value-based, patient-centered primary care
- • Ability to work collaboratively within a team-based care model
Preferred
- • Experience practicing in a value-based or capitated care model
- • Prior work with senior-focused or Medicare patient populations
- • Interest in preventive care, population health, and chronic disease management
- • Comfort working within an interdisciplinary care team
- • Experience with Athena or comparable EMR systems
