The role provides hands-on, patient-centered medical care for residents in a skilled rehabilitation and long-term care setting. It includes clinical decision-making, interdisciplinary collaboration, supervision of mid-level providers or limited permit physicians as applicable, and ensuring regulatory and quality standards are met.
Key Responsibilities
- • Provide comprehensive medical care to residents in skilled rehab and long-term care
- • Conduct initial evaluations, routine follow-ups, and acute change-in-condition assessments
- • Manage chronic and complex medical conditions
- • Oversee admissions, readmissions, and discharge planning
- • Review and sign orders, progress notes, and care plans
- • Collaborate with nursing leadership, rehabilitation therapists, social work, and administration
- • Participate in interdisciplinary care plan meetings
- • Communicate effectively with residents and families
- • Supervise and mentor limited permit physicians, nurse practitioners, or physician assistants (as applicable)
- • Ensure compliance with state, federal, and facility regulations
- • Support quality improvement initiatives and best clinical practices
- • Coordinate with hospitals, specialists, and outside providers as needed
Required
- • MD or DO from an accredited medical school
- • Active New York State Medical License
- • Board Certified or Board Eligible in Internal Medicine
- • Board Certified or Board Eligible in Family Medicine
- • Strong clinical judgment and decision-making skills
- • Excellent communication and documentation abilities
- • Ability to work collaboratively within an interdisciplinary team
- • Comfort working in a skilled nursing and rehabilitation environment
Preferred
- • Geriatrics (preferred, not required)
- • Prior experience in skilled nursing facilities
- • Prior experience in post-acute or rehabilitation settings
- • Prior experience in long-term care
- • Familiarity with EMR systems
- • Experience supervising mid-level providers or limited permit physicians

We are seeking a compassionate, experienced Attending Physician to join our medical team and provide hands-on, patient-centered care in a skilled nursing and rehabilitation setting.
Position Overview
The Attending Physician is responsible for overseeing and managing the medical care of residents in the skilled rehabilitation and long-term care setting. This role includes clinical decision-making, collaboration with nursing and rehab teams, supervision of mid-level providers and limited permit physicians (if applicable), and ensuring regulatory and quality standards are met.
This position is ideal for physicians interested in geriatrics, post-acute care, internal medicine, or family medicine within a team-based environment.
