Attending Physician MD DO

The role provides hands-on, patient-centered medical care for residents in a skilled rehabilitation and long-term care setting. It includes clinical decision-making, interdisciplinary collaboration, supervision of mid-level providers or limited permit physicians as applicable, and ensuring regulatory and quality standards are met.

Key Responsibilities
  • Provide comprehensive medical care to residents in skilled rehab and long-term care
  • Conduct initial evaluations, routine follow-ups, and acute change-in-condition assessments
  • Manage chronic and complex medical conditions
  • Oversee admissions, readmissions, and discharge planning
  • Review and sign orders, progress notes, and care plans
  • Collaborate with nursing leadership, rehabilitation therapists, social work, and administration
  • Participate in interdisciplinary care plan meetings
  • Communicate effectively with residents and families
  • Supervise and mentor limited permit physicians, nurse practitioners, or physician assistants (as applicable)
  • Ensure compliance with state, federal, and facility regulations
  • Support quality improvement initiatives and best clinical practices
  • Coordinate with hospitals, specialists, and outside providers as needed
Required
  • MD or DO from an accredited medical school
  • Active New York State Medical License
  • Board Certified or Board Eligible in Internal Medicine
  • Board Certified or Board Eligible in Family Medicine
  • Strong clinical judgment and decision-making skills
  • Excellent communication and documentation abilities
  • Ability to work collaboratively within an interdisciplinary team
  • Comfort working in a skilled nursing and rehabilitation environment
Preferred
  • Geriatrics (preferred, not required)
  • Prior experience in skilled nursing facilities
  • Prior experience in post-acute or rehabilitation settings
  • Prior experience in long-term care
  • Familiarity with EMR systems
  • Experience supervising mid-level providers or limited permit physicians

We are seeking a compassionate, experienced Attending Physician to join our medical team and provide hands-on, patient-centered care in a skilled nursing and rehabilitation setting.

Position Overview

The Attending Physician is responsible for overseeing and managing the medical care of residents in the skilled rehabilitation and long-term care setting. This role includes clinical decision-making, collaboration with nursing and rehab teams, supervision of mid-level providers and limited permit physicians (if applicable), and ensuring regulatory and quality standards are met.

This position is ideal for physicians interested in geriatrics, post-acute care, internal medicine, or family medicine within a team-based environment.

Qualifications & Requirements

  • MD or DO from an accredited medical school
