Key Responsibilities
- • Manage patient encounters averaging 18-22 patients per day

- • Respond to low intensity call frequency (1:6) with phone consultation only with first tier nurse triage
Required
- • Graduate of an accredited ACGME/AOA Internal Medicine residency training program
- • BC/BE with the ABMS or AOA
- • Licensed or ability to obtain licensure through the Minnesota Board of Medical Practice
- • Ability to obtain and/or maintain DEA certification for the State of Minnesota
- • Meet threshold criteria/qualifications for Credentialing and Privileges if applicable

Practice Details
- 9 provider primary care practice which includes Family Medicine, Internal Medicine, Pediatrics, and Nurse Practitioners and Physician Assistants
- Specialists in sports & physical therapy, anticoagulation, medication therapy management, and an onsite pharmacy.
- Patient encounters average 18-22 patients per day.
- Virtual care offered and encouraged via video, telephone, and through EMR.
- 34 patient contact hours per week for full time - flexible schedule.
- Call Frequency: 1:6 low intensity - phone consultation only with first tier nurse triage
- As a Pioneer Accountable Care Organization (ACO), our physicians use a multidisciplinary team approach in managing clinical care.
- Fully integrated Electronic Medical Record (EPIC).
M Health Fairview Clinic - Rosemount is located just 15 south of the Twin Cities metropolitan area and predominantly serves the communities of Eagan, Rosemount, Apple Valley, and Inver Grove Heights.

Qualifications

The posted pay range is for a 1.0 FTE. The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions. Additionally, our organization values pay equity and considers the internal equity of our team when making any offer. Positions with the same min/max are flat rate jobs based on 1.0FTE schedule. Compensation plans based on production can allow for higher pay than the range posted.
