*Southside Outpatient Primary Care Opportunity with IU Health in the home of Purdue University
The physician will practice team-based care in a progressive family medicine clinic, focusing on chronic disease management and wellness for patients of all ages. The role involves a full-time outpatient schedule with participation in process improvement within a collegial team environment.
Key Responsibilities
- • Practice team-based care in a family medicine clinic
- • Manage chronic diseases and provide wellness care for all ages of patients
- • Participate in process improvement
- • Provide care in a 100% outpatient setting
- • Perform procedures in onsite procedure suites and lab
- • Share phone call approximately 1:25 for family medicine
Required
- • US BE/BC in Family Medicine, Internal Medicine or Internal Medicine/Pediatrics
- • MD/DO Indiana license or eligible
- • Consistent and consecutive employment/training history
- • Excellent communication skills
- • Ability to provide high quality patient care
- • Team oriented, positive and patient centered

- Shared phone call only: approximately 1:25 for FM 1:12 IM
