The Physician Medical Director for the COMPASS Program combines direct clinical care with program leadership, maintaining an active patient panel while providing medical oversight across senior living communities and home-based settings. This role involves guiding program operations, clinical quality, and strategic development to improve patient experience and reduce hospital utilization.
Key Responsibilities
- • Maintain a primary patient panel of approximately 150–200 COMPASS patients
- • Provide medical oversight for an additional ~200 patients, with anticipated program growth
- • Deliver evidence-based, patient-centered care to medically complex and aging populations
- • Support complex clinical decision-making and goals-of-care discussions
- • Participate as backup Medical Director supporting APC on-call coverage as needed
- • Serve as Medical Director of the COMPASS Program, providing overall clinical leadership
- • Collaborate closely with the COMPASS Program Manager and Clinical Liaison
- • Provide clinical escalation support and guidance to nursing and provider teams
- • Partner with executive leadership to support program growth and innovation
- • Develop and refine clinical protocols, workflows, and care pathways
- • Lead quality improvement initiatives focused on outcomes, continuity, and hospital avoidance
- • Monitor performance metrics and guide continuous improvement efforts
- • Ensure compliance with regulatory and organizational standards
- • Work closely with NPs, PAs, nurses, care coordinators, and operational leaders
- • Serve as a clinical mentor and resource to program staff
- • Support alignment between clinical operations and organizational leadership
- • Promote longitudinal, relationship-based care models
- • Support patients and families in understanding care plans and goals of care
- • Champion dignity, access, and continuity in care delivery
Required
- • MD or DO degree required
- • Board certification in Internal Medicine, Family Medicine, Geriatrics, or related specialty
- • Active, unrestricted Pennsylvania medical license; willingness to obtain additional state licenses as program expands
- • Experience caring for aging, palliative, or medically complex populations
- • Comfortable delivering care in a remote or virtual environment
- • Proven collaboration within interdisciplinary care teams
- • Strong clinical judgment and patient-centered approach
- • Effective leadership, communication, and mentoring skills
- • Ability to balance clinical care with program leadership responsibilities
- • Proficiency with EMRs and virtual care platforms
- • Organized, adaptable, and comfortable in a growing, dynamic healthcare environment

Position Overview
The Physician Medical Director for the COMPASS Program is a fully remote, dual-role position combining direct clinical care with program leadership. This physician leader maintains an active patient panel while providing medical oversight for a broader COMPASS population across senior living communities and home-based settings. The COMPASS Program (Collaborative Outpatient Management for Palliative and Aging Support Services) is central to this mission, providing coordinated, longitudinal care that reduces fragmentation and unnecessary hospital utilization while improving patient experience and quality of life.
In partnership with the COMPASS Program Manager and Clinical Liaison, the Medical Director guides program operations, clinical quality, and strategic development. While the Program Manager oversees day-to-day nursing operations, the Medical Director provides clinical leadership, escalation support, and program direction.
