Legacy Care
The Physician will deliver comprehensive primary and acute medical care to residents in post-acute and long-term care facilities, performing assessments, diagnosis, treatment, medication management, and collaborating with interdisciplinary teams. This PRN role requires flexibility to cover variable shifts and a commitment to person-centered, evidence-based practice.
Key Responsibilities
- • Perform comprehensive assessments and develop individualized care plans for residents in post-acute and long-term care settings
- • Diagnose, treat and manage acute and chronic medical conditions consistent with current evidence-based practice
- • Provide medication management, including review of polypharmacy and coordination of medication reconciliation
- • Respond promptly to acute changes in condition, initiate timely interventions and coordinate transfers of care when necessary
- • Collaborate with nursing staff, advanced practice clinicians, therapists, social workers and facility leadership to deliver holistic, person-centered care
- • Participate in facility rounds, care-planning meetings and interdisciplinary case conferences
- • Document clinical encounters accurately and promptly in the Electronic Medical Record (EMR)
- • Support medication safety initiatives, including antimicrobial and antipsychotic stewardship
- • Provide clinical consultation and education to facility staff and medical colleagues on complex clinical issues and best practices
- • Maintain professional standards of practice, including infection control, documentation and regulatory compliance
Required
- • Doctor of Medicine (MD) or Doctor of Osteopathy (DO) from an accredited institution
- • Active and unrestricted Virginia medical license
- • Proficiency with Electronic Medical Record systems and clinical documentation
- • Strong clinical judgement, interpersonal and communication skills; ability to work effectively within multidisciplinary teams
- • Commitment to patient-centered care, cultural sensitivity and ethical practice
- • Current Basic Life Support (BLS) certification
Preferred
- • Board certification or eligibility in an appropriate specialty (for example family medicine, internal medicine, geriatrics)
- • Clinical experience in post-acute care, long-term care, geriatrics or related settings
- • Advanced Cardiac Life Support (ACLS) preferred where applicable
