This role involves conducting telehealth bone health evaluations for patients with osteoporosis or osteopenia, focusing on evidence-based assessment, risk documentation, and transitioning eligible patients to a PT-led remote therapeutic monitoring exercise program. The physician will engage in structured consults, patient education, and contribute to scalable clinical workflows.
Key Responsibilities
- • Perform telehealth bone health consults (e.g., osteoporosis/osteopenia risk, fragility fracture history, secondary causes screening prompts).
- • Review relevant history: fractures/falls, family history, medications (e.g., steroids), nutrition, activity level, endocrine risk factors, renal/liver disease, GI malabsorption, smoking/alcohol, etc.
- • Interpret existing data when available (DEXA, labs, imaging reports) and document clinical impression and risk level.
- • Recommend appropriate follow-up orders as needed (e.g., DEXA or lab evaluation)— we currently do not prescribe medication
- • Provide patient-facing education: bone-friendly lifestyle, calcium/vitamin D guidance (general education), fall prevention, strength/balance training rationale, and red flags that warrant escalation.
- • Determine candidacy for a PT-led exercise plan under RTM and document medical necessity for monitoring when appropriate.
- • Create a clear handoff note to PT: goals, precautions/contraindications, comorbidities, fall-risk tier, and recommended exercise focus areas.
- • Collaborate with operations to refine workflows, templates, and quality measures.
- • Help develop clinical protocols, patient education materials, escalation pathways, and QA review.
- • Participate in periodic case conferences with PT/clinical team.
Required
- • MD or DO with an active, unrestricted U.S. medical license in California
- • Board eligible/certified in Family Medicine, Internal Medicine, Geriatrics, Endocrinology or related.
- • Comfort with telehealth workflows, technology, and structured documentation.
- • Strong patient communication and counseling skills.
Preferred
- • Multi-state Licensure
- • Experience with osteoporosis/bone health, falls/fracture prevention, geriatrics, or metabolic health.
- • Familiarity with rehabilitation/exercise medicine collaboration.

Role Overview
We’re hiring a part-time, remote physician to conduct bone health evaluations for patients with osteoporosis/osteopenia. You’ll perform an evidence-based telehealth assessment, document risks and plan, and transition eligible patients to a PT-led RTM exercise program with clear medical oversight and follow-up checkpoints. This is ideal for a physician who enjoys structured consults, patient education, and building scalable clinical workflows.
Logistics
- Hours: Part-time (5-20 hours/week), flexible blocks, remote.
- Schedule: Ability to make at least 20 hours per week available for scheduling. Actual clinical hours may vary based on demand
- Pay: $120-180 / hour
- Visit format: Telehealth visits (typical 20 minutes) + light asynchronous coordination.
- Patient population: Patients with at risk for or with confirmed osteopenia/osteoporosis; history of balance issues or falls; post-fracture patients when applicable.
