The physician will provide primary care to patients across the generational spectrum with a focus on improving wellness for homeless and low-income residents. The role involves integrated behavioral health services, managing chronic diseases with nurse case managers, and supporting patients with mental health and substance use disorders, including LGBT-aware and transgender healthcare.
Key Responsibilities
- • Care for patients across the generational spectrum
- • Improve overall wellness of homeless and low-income residents regardless of resources or ability to pay
- • Provide full-service primary care with after-hours on-call nurse triage for urgent needs
- • Work within an integrated behavioral health model to serve populations seeking mental health services
- • Develop patient-centered care plans
- • Provide support and education with a nurse case manager specializing in chronic disease management
- • Support individuals and family members struggling with mental health disorders, including substance use disorder and addiction
- • Help connect patients with substance use disorder services in the community
- • Provide LGBT-aware care with on-site transgender healthcare
- • Work closely with homeless populations and newly arrived refugees
