Provide assessment and consulting on patients at two area skilled nursing facilities, offering medical oversight of patient care and rehabilitation. Collaborate with nurse practitioners and home care teams, with opportunities for medical direction, leadership, research, education, and clinical program development.

Required
- • Candidates must be board certified/eligible in Internal Medicine, Family Medicine or Geriatric Medicine.
Preferred
- • Achieving CMD certification will be supported.
