Position Summary

This is a part-time Family Medicine Physician opportunity based at our Chicago Heights location, with the option to transition to full-time. The Physician will provide comprehensive, patient-centered primary care while supporting efficient clinic operations.

Essential Duties and Responsibilities

• Practice within the scope of evidence-based clinical guidelines

• Provide continuous, comprehensive family medicine and primary care services for infants, children, adolescents, and adults in accordance with current standards of care

• Obtain and document patient medical histories

• Perform and/or order diagnostic tests

• Diagnose conditions and prescribe medications as appropriate

• Administer vaccinations and conduct follow-up examinations

• Educate patients on disease prevention and health maintenance

• Prepare and maintain accurate medical records, clinical reports, and correspondence

• Support quality improvement initiatives

• Collaborate with the management team and clinical providers

• Complete all required Core Curriculum training for the role

• Perform other duties as assigned

Qualifications

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below represent the knowledge, skills, and abilities required.

Education and Experience

• Graduate of an accredited medical school with a Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree

• Completion of an Internship and an accredited Family Medicine Residency Program

Licensure & Additional Requirements

• Current and unrestricted medical license in the state of Illinois

• Current and unrestricted Drug Enforcement Administration (DEA) certificate

• SCIS Certified Civil Surgeon (HP) – required

• Valid driver’s license

• Current auto insurance

• Completion of all required Continuing Medical Education (CME)
