Medical Director & Faculty Physician

This role involves supervising healthcare providers at a designated health center, providing direct patient care, and fostering resident development through precepting and structured teaching. The position also includes shaping the residency curriculum and mentoring residents under the guidance of the Family Medical Residency Director.

Key Responsibilities
  • Provide medical services to patients in accordance with CHN clinical policies and procedures on a regularly assigned schedule.
  • Precept residents in the clinic as scheduled, adhering to ACGME residency requirements.
  • Maintain clear, concise, timely, and legible documentation of patient care and resident attestation notes using the Electronic Health Record platform.
  • Write, review, revise, and monitor assigned curricular areas, as well as the overall curriculum, at least annually with the program director.
  • Act as an advisor for assigned residents, meeting with them at least quarterly in accordance with protocol, presenting resident advisees quarterly or preparing documentation for the designee, and developing individualized learning plans.
  • Prepare and present didactic lectures, lead group discussions, and plan continuing medical education sessions.
  • Serve on committees and attend meetings at the clinic and/or hospital as requested or required by CHN’s Medical Affairs and relevant policies.
  • Fulfill Accreditation Council for Graduate Medical Education (ACGME) requirements for core family medicine faculty.
  • Participate in faculty development and meetings.
  • Apply knowledge of medical terminology and CPT/ICD billing codes to deliver effective and efficient patient care.
  • Assist with the interviewing, selection, and ranking of prospective residency candidates.
  • Mentor residents in scholarly activities, quality improvement projects, poster presentations, and publications.
  • Conduct comprehensive examinations and diagnoses of patients at the assigned center.
  • Propose policies on care issues and conduct chart audits to ensure compliance.
  • Collaborate with leadership staff at the assigned Health Center in all aspects of center operations.
  • Participate in monthly clinical staff meetings and provider meetings.
  • Oversee the chart audit process and maintain a peer review process at the assigned center.
  • Facilitate related services for health center patients and CHN staff, ensuring confidentiality and privacy.
  • Engage in continuing education and maintain credentials as required.
  • Participate in committees and perform other duties as assigned by the supervisor.
  • Review and interpret medical history data, continually updating this information as necessary.
  • Perform other related duties as assigned by supervisor.
Required
  • Medical degree from an accredited medical school
  • Currently licensed to practice medicine New York State
  • Board Certified in Family Medicine
Preferred
  • Prior FQHC clinical experience and/or experience working with a medically underserved populations preferred
  • Prior teaching experience is preferred

Position Summary

Under the direction of the Chief Medical Officer/Vice President of Medical Affairs, this role encompasses the supervision of healthcare providers at the designated health center, including but not limited to physicians, nurse practitioners, certified nurse midwives, and physician assistants. Furthermore, this role provides direct patient care in a clinical setting while fostering the development of residents through precepting and structured teaching. Under the collaborative guidance of the Family Medical Residency Director, the position plays a crucial role in shaping the curriculum and mentoring residents, ensuring the successful execution of Community Healthcare Network's mission.
