This role involves providing comprehensive, high-quality primary care to senior patients in state-of-the-art community centers. Physicians work with small patient panels supported by a fully supportive care team, focusing on building relationships and addressing both medical and social needs.
Key Responsibilities
- • Deliver better primary care to patients with a focus on quality rather than volume
- • Spend more time with patients to provide comprehensive care
- • Be available to patients and address their medical and social needs
- • Lead care teams with a patient-centric orientation
- • Care for patients with comorbid conditions, the elderly, and/or medically underserved
Required
- • M.D. or D.O. graduates
- • Board certified/eligible in Internal Medicine or Family Medicine
- • Have an active, non-probationary state medical license
- • Be highly collaborative and enjoy leading care teams with a patient-centric orientation
- • Confident in ability to deliver high quality primary care
- • Driven to care for those with comorbid conditions, the elderly and/or medically underserved
- • Passionate about their work and energized by being part of a fast growing organization
Preferred
- • Fellowship training in Geriatrics welcome, but not required
