Family Practice Physician (31859)

The Family Practice Physician provides primary medical care to pediatric and adult patients in ambulatory and in-patient settings, focusing on illness prevention and health promotion. The role involves collaboration with Adult Advance Practice Nurses, leadership and training in Internal Medicine, and participation in health program planning within the Health Center and community.

Key Responsibilities
  • Provide primary medical care services to pediatric and adult patients in both ambulatory and in-patient settings using primary care concepts.
  • Emphasize prevention of illness and the promotion of healthy lifestyles.
  • Collaborate with Adult Advance Practice Nurses in the preparation and implementation of patient care protocols.
  • Provide leadership and training in Internal Medicine to other health care professionals.
  • Participate in the program planning of health activities within the Health Center and in the community, as requested.
  • Follow role assignments aligned with Patient Centered Medical Home Standards.
  • Participate in all Patient Centered Medical Home efforts.
Required
  • Graduate of an Accredited Medical School.
  • Current Illinois State License, Controlled Substance License & DEA.
  • Licensed as a Physician in the State of Illinois.
  • Completed an Accredited Post-Graduate Training Program in Family Medicine.
  • Board Certified or Eligible in Family Medicine.

Your Role
 

Family Practice Physician provides primary medical care services to pediatric and adult patients in both the ambulatory and in-patient setting using primary care concepts, emphasizing prevention of illness and the promotion of health lifestyles. Collaborates with the Adult Advance Practice Nurses in the preparation and implementation of patient care protocols.  Provides leadership and training in Internal Medicine to other health care professionals.  Participates in the program planning of health activities within the Health Center and in the community, as requested.

Will follow role assignments that are in alignment with the Patient Centered Medical Home Standards, and participate in all Patient Centered Medical Home efforts.
 

 

Qualifications

 

Education Requirements:   Graduate of an Accredited Medical School.
Required Licensure and/or Certification:  Current Illinois State License, Controlled Substance License & DEA.
Minimum Experience: Licensed as a Physician in the State of Illinois.  Completed an Accredited Post-Graduate Training Program in Family Medicine.  Board Certified or Eligible in Family Medicine.
