An older adult gets discharged from the hospital—you already know what happens. The family scrambles. Nobody talks to each other. Medications get confused. A fall happens. Another admission. The system wasn't designed for this moment. We started Hera because we felt it wasn't enough.

Hera is dedicated care management in the community for older adults, covered by Medicare. Every family gets matched with a dedicated Hero (care manager) who has the tools, time, and autonomy to deliver ongoing support. You're the clinical entry point—the comprehensive assessment that sets the clinical foundation for real, ongoing intervention.

The Role

We're building the clinical infrastructure that makes great geriatric care exceptional. As a Clinical MD, you'll conduct telehealth assessments that serve as the clinical gateway for families newly enrolled with Hera. Each 45-60 minute visit establishes the clinical foundation: confirming Medicare eligibility, synthesizing complex medical and social history, identifying barriers to treatment, and flagging clinical priorities for our care managers (Heroes) to address over months of ongoing coordination. This assessment work directly shapes the care strategy that families receive.

Key Responsibilities

• Conduct comprehensive telehealth assessments for newly enrolled Medicare clients
• Synthesize complex medical histories and identify clinical priorities for ongoing care coordination
• Document clinical findings that inform care manager (Hero) intervention strategies
• Provide clinical oversight and collaboration as needed with care management team
• Establish clinical credibility and trust during initial patient contact

Requirements

• MD or DO (must be licensed to practice in: MD, PA, MA, CA, FL, or NJ)
• Substantial experience in geriatric medicine, primary care, or chronic disease management
• Confident in telehealth assessment and skilled at building clinical rapport across a screen
• Strong clinical judgment and comfort with complexity and ambiguity

Compensation & Hours

• $100 - $175/hr depending on license type, location, and experience
• 3-10+ hours per week to support expected case load (flexible)
• Preferred: Weekday 11am-4pm ET, but can self-select working hours
