Primary Care Physician
Provide supervisory and clinical oversight for Long-Term Care and Skilled Nursing Facilities in rural Southeast Colorado. Manage chronic and complex geriatric patients, collaborate with facility teams, and participate in quality improvement and post-acute care initiatives. Required
- • MD or DO
- • Board Certified/Eligible in Internal Medicine, Family Medicine, or Geriatrics
- • Active CO medical license (or ability to obtain)
- • DEA license
Preferred
- • Experience in LTC, SNF, geriatrics, post‑acute care, or rural medicine
- • Familiarity with Medicare and value‑based care
- • Interest in leadership or future program growth
