Remote Pulmonologist NY – Telemedicine | Long-Term Care & Nursing Home | Supervising Role

Provide consultative respiratory care for residents in nursing homes and long-term care facilities through telemedicine and on-site services. Support and mentor Advanced Practice Providers, supervise Nurse Practitioners, collaborate with interdisciplinary teams, and participate in quality improvement initiatives.

Key Responsibilities
  • Support and mentor Advanced Practice Providers to ensure care is delivered in alignment with clinical protocols, regulatory requirements, and quality standards.
  • Supervise and provide clinical oversight to Nurse Practitioners delivering pulmonary services in nursing homes, including case review, clinical guidance, and collaboration on complex patients.
  • Collaborate with facility medical directors, nursing leadership, and interdisciplinary care teams.
  • Participate in quality improvement initiatives, care pathway development, and clinical protocol optimization.
  • Engage in telemedicine consultations and periodic on-site visits as needed.
  • Provide pulmonary consultations, evaluations, and ongoing management for residents in skilled nursing facilities and long-term care settings.
  • Interpret pulmonary function tests, imaging, and other diagnostic studies.
  • Develop and oversee individualized, evidence-based treatment plans to improve outcomes and reduce hospital readmissions.
Required
  • MD or DO with completed Pulmonary fellowship; board-certified or board-eligible.
  • Active, unrestricted medical license in New York.
  • DEA registration.
  • Ability to work independently while collaborating within a multidisciplinary care model.
  • Supervising/collaborating with NPs or PAs: 1 year
  • Board-certified or board-eligible in Pulmonary Medicine
  • Active DEA registration
  • Active and unrestricted Pennsylvania medical license
Preferred
  • Experience in post-acute care, long-term care, or consultative pulmonary services preferred.
  • Prior experience supervising or collaborating with Nurse Practitioners or Advanced Practice Providers strongly preferred.
  • Telemedicine consultations: 1 year

Compensation: $2,000 per diem + bonus
