This role involves delivering full-spectrum primary care with a focus on individualized attention through managing a smaller patient panel. The physician will provide proactive preventive care, coordinate with specialists, and offer extended appointment durations along with 24/7 patient access through various communication methods.
Key Responsibilities
- • Deliver full-spectrum primary care, including preventive services, diagnosis, treatment, and management of acute and chronic health conditions across all age groups.
- • Manage a smaller, dedicated patient panel to ensure individualized attention and easy access to care.
- • Perform detailed patient evaluations, including physical exams and thorough medical histories.
- • Create tailored care plans emphasizing long-term wellness and health maintenance.
- • Focus on proactive preventive care such as screenings, immunizations, and lifestyle guidance.
- • Collaborate with specialists and other healthcare providers to coordinate seamless patient care.
- • Provide prompt appointments, offering same-day or next-day availability to meet patient needs.
- • Offer after-hours availability through phone, secure messaging, or telemedicine for patient support.
- • Educate patients on disease prevention, health promotion, and wellness strategies.
- • Maintain precise and current electronic medical records, adhering to all documentation standards.
Required
- • Board certified in Family or Internal Medicine
- • Valid and active medical license in the state of Florida
Preferred
- • 5 years experience in an outpatient concierge or primary care setting preferred
- • Hormone therapy experience is a plus

- Availability for same-day or next-day visits, averaging 36 clinical hours weekly.
- Average of six patient visits daily, conducted in-person, virtually, or occasionally in home or skilled nursing facility settings.
- Development of individualized health plans to prevent and manage chronic illnesses.
- Emphasis on wellness and preventive health, including routine screenings and proactive care strategies.
- Coordination with specialists to ensure integrated and continuous care.
- Board certified in Family or Internal Medicine
